The kidney is an important organ of the human body, its main function is filtration through the glomerulus and reabsorption by the renal tubules and secretory functions to generate urine, excretion of metabolic wastes, maintenance of metabolic balance such as water and electrolyte acid-base in the body; kidneys also have endocrine functions (such as the production of renin, erythropoietin, active vitamin D, etc.) to regulate blood pressure, erythropoiesis and calcium Phosphorus metabolism.

So, which diseases require kidney function tests? What are the results of renal function?

Which diseases require renal function tests?

Monitoring of renal function is required in the presence of renal disease such as glomerulonephritis, nephrotic syndrome, renovascular disease, severe or recurrent urinary tract infections, urinary tract stones or tumors.

Kidney function screening is also required when some other systemic diseases outside the kidney may affect the kidney, such as certain congenital or genetic diseases that may affect the kidney, heart disease or hypertension, viral hepatitis or cirrhosis and other liver diseases, allergic reactions or allergic purpura, endocrine system diseases such as diabetes or gout, and rheumatic system diseases, especially systemic lupus erythematosus, vasculitis, rheumatoid arthritis, dermatomyositis, etc. In addition, the use of certain drugs that may damage renal function should also pay attention to re-examination of renal function.

How about the results of renal function?

The “renal function” that everyone is most concerned about actually refers to the glomerular filtration function, which is expressed by the glomerular filtration rate (GFR). Accurate assessment of GFR is of great significance for correctly judging the staging of chronic kidney disease, evaluating the rate of progression of renal function and the effect of intervention therapy, and judging the timing of starting renal replacement therapy.

The gold standard for evaluating glomerular filtration rate is inulin clearance rate, and clinically, it is mainly measured by serum urea, serum creatinine, serum cystatin C and estimated glomerular filtration rate. value to assess renal function.

• Serum urea (Urea)

Serum urea is the end-stage product of human protein and amino acid metabolism, with a small molecular weight. Glomerular filtration, under normal circumstances, about 30~40% is reabsorbed by the renal tubule, and its concentration can reflect the filtration function of the glomerulus to a certain extent, but only when the glomerular filtration function drops below 50% , the serum urea concentration will increase.

In addition, extrarenal factors can also cause elevated serum urea. High-protein diet, gastrointestinal bleeding, infection, etc. can increase serum urea; while low-protein diet, drinking more water, and urination can lead to decreased serum urea.

• Serum creatinine (Cr)

Creatine is the end-stage metabolite of creatine. Creatinine is a small molecule that is filtered through the glomerulus, and the renal tubules do not substantially reabsorb creatinine in the original urine. Serum creatinine concentration and urinary creatinine excretion are indicators to evaluate glomerular filtration function.

In general, creatinine will exceed the upper limit of its reference range only when the glomerular filtration rate falls below 50%. At the same time, serum creatinine is also affected by factors such as gender, age, muscle mass, and protein intake.

• serum cystatin C (Cys-C)

serum cystatin C is an ideal endogenous index reflecting glomerular filtration rate . Serum cystatin C is cleared by glomerular filtration and reabsorbed in the proximal convoluted tubule; after reabsorption, it is completely metabolized and decomposed and does not return to the blood. Therefore, the concentration of serum cystatin C in the blood is determined by the kidneys. The pellet filtration decision does not depend on any external factors such as gender, age, diet.

The diagnostic sensitivity of serum cystatin C is better than that of traditional renal function tests such as creatinine and blood urea nitrogen, which is suitable for the early diagnosis of renal disease.

• Estimated glomerular filtration rate (eGFR)

GFR refers to the amount of filtrate produced by both kidneys per unit time , mainly to remove harmful substances in the blood to the body. The normal adult glomerular filtration rate is (125 ± 15) ml/min in men and about 10% lower in women. Clinically, the staging of chronic kidney disease is based on the value of eGFR.
