Recently, our hospital admitted a patient with ruptured tophi complicated by infection. The patient went to the outpatient clinic of Dr. He Yi, deputy director of the Department of Rheumatology and Immunology in our hospital. A preliminary examination found that there were tophi ulcers and exudation in multiple joints such as the patient’s toes and ankles. After treatment in many local hospitals, the condition could not be effectively controlled. Deputy Chief Physician He Yi immediately contacted Chief Physician Gu Zhengtao of the trauma treatment center of our hospital for a multidisciplinary consultation. It was considered that the patient had tophi rupture with infection, and the skin defect was severe, and further hospitalization was required. The patient was admitted to the trauma treatment center of our hospital. After a series of inspections, it was found that the patient not only had tophi rupture, but also the distal bone of the fourth toe had been eroded by tophi. The team of chief physician Gu Zhengtao formulated a detailed treatment plan for the patient: thorough debridement, anti-infective treatment, and pedicled skin flap transplantation; during this period, the Department of Rheumatology and Immunology formulated an effective adjuvant treatment plan for preventing gout attacks and controlling uric acid. The wound healed and was discharged.
Wound healing is only the first step, and regular uric acid-lowering therapy will be the key to future gout recovery.
