Recently, a rare blood infection caused by Yersinia enterocolitica, with liver, lung and bone Ms. Qiu, who suffered from joint soft tissue abscess, finally recovered and was discharged from the hospital after receiving 36 days of individualized and meticulous treatment in the geriatric ward of the Qunli District of the First Hospital of Harbin Medical University.
Before the Spring Festival in 2022, 57-year-old Ms. Qiu was diagnosed with multiple space-occupying liver lesions in a local hospital, and she was transferred to the Department of Geriatrics of the Qunli Hospital of the First Hospital of Harbin Medical University for treatment. On admission, the patient was in critical condition, lethargic, with a body temperature of 39.6°C and low blood pressure. Professor Wu Xiuping, director of the Department of Geriatrics, gave the patient active anti-infection and other supportive treatments, and continued to improve relevant examinations.
On the second day of admission, the patient had sudden chills, body temperature as high as 40°C, blood pressure only 80/40mmHg, heart rate 130 beats/min, and blood oxygen saturation as low as 90%. The patient developed septic shock. Both blood bacterial culture and liver abscess puncture and bacterial culture were reported as Yersinia enterocolitica.
The medical staff of the geriatrics department immediately rescued him and gave life support measures. The patient was diagnosed as “sepsis, abscess formation in multiple organs” , and developed an individualized treatment plan for patients with anti-infection, nutritional support, stable internal environment, stable blood pressure and blood sugar. After a week of symptomatic treatment, although Ms. Qiu’s body temperature remained high, her condition gradually stabilized, and the liver abscesses, abscesses around the hip joints, and multiple abscesses in the lungs were gradually absorbed.
Professor Wu Xiuping pointed out that Yersinia enterocolitica is common in nature and can produce heat-resistant enterotoxin. Eating food contaminated with this bacteria often causes enteritis and diarrhea. However, if the disease enters from the mouth, the bacteria can also grow at about 4°C. If food that has been refrigerated for a long time is not thoroughly heated before eating, there is also a risk of infection with the bacteria. Liver abscess is a purulent infection caused by bacteria and other pathogens invading the liver, which can cause sepsis, septic shock, etc., and the fatality rate can be as high as 10%-30%. Yersinia enterocolitica has a high fatality rate and aggressiveness, and co-infection of the blood, liver, lungs, and bone and joint soft tissue is indeed rare.
