what is insulin resistance
Insulin is a peptide hormone secreted by the pancreatic islet beta cells. It is the only hormone in the body that lowers blood sugar and is the only hormone that simultaneously promotes the synthesis of glycogen, fat and protein. Under normal circumstances, islet β-cells will sense changes in the body’s blood sugar concentration. For example, when blood sugar rises during meals, it will secrete more insulin. On the contrary, when blood sugar is low, it will secrete less insulin, so as to maintain the dynamic balance of blood sugar.
Insulin resistance refers to the reduced responsiveness (ie insensitivity) of the body to the physiological action of insulin, mainly manifested in the uptake and utilization of glucose by insulin targets such as liver, skeletal muscle and adipose tissue obstacle.
How to identify insulin resistance
If you want to judge whether you have insulin resistance, then you need to go to a regular medical institution to do these two tests – oral glucose tolerance test (OGTT) and insulin release test (INS). The two trials measured fasting blood glucose levels and insulin levels after quantitative oral glucose to understand islet beta cell function and the body’s ability to regulate blood sugar.
Notes on Assays
Before the test
No overeating or excessive exercise for at least three days.
Fasting for 8-14 hours, recommended fasting for 10-12 hours. Drink water (sugar-free) and brush your teeth during the period.
The hospital usually measures in the morning. It is recommended to go to the hospital for examination on an empty stomach in the morning.
Testing
Drink within 5 minutes and dissolve in 200-300ml of water 75g anhydrous glucose powder.
Do not leave the hospital on the way, do not engage in strenuous activity, drink any beverages, but drink a small amount of water.
Advanced fasting blood collection, then start timing from drinking the first sip of sugar water, and draw venous blood to check blood sugar at 30 minutes, 60 minutes, 120 minutes and 180 minutes respectively.
how to interpret the results

(1) Interpretation of OGTT results: 0 hours: 3.9~6.1mmol/L; 0.5~1 hours: <11.1mmol/L; 2 hours: 3.3~7.7mmol/L; 3 hours: 3.3~5.5mmol/L Blood glucose less than 7.8mmol/L 2 hours after taking sugar is in the normal range and cannot be diagnosed with impaired glucose tolerance; blood sugar >11.1mmol/L 1 hour after taking sugar should be treated as a high-risk diabetic patient, and insulin should be further measured Release test to observe whether there is a delay in the peak of insulin secretion.
(2) Interpretation of INS results: fasting insulin is within the normal range, and the peak insulin secretion after taking sugar is 5-10 times the initial value; Within hours, insulin levels peaked; within 3 hours of fasting, they returned to the normal range.
Note: It is also problematic if the glucose tolerance results are normal and the insulin release test results are abnormal.
