The award The primary objective of the study was to compare the perioperative safety and long-term outcomes between minimalist one-stop surgery guided by DSA alone and traditional one-stop surgery guided by esophageal ultrasound (TEE). The study included 138 non-valvular atrial fibrillation (AF) patients who underwent catheter ablation + left atrial appendage closure one-stop operation in Shanghai Tenth People’s Hospital from February 2019 to December 2020. The occurrence of postoperative complications and long-term clinical prognosis.
The results of the study showed that there was no significant difference in fluoroscopy time between the minimalist group and the traditional group (15.2±5.4 minutes vs. 14.4±7.1 minutes, P=0.074), The minimalist group had significantly less operation time than the traditional group (95.7±27.6 minutes vs 108.9±30.3 minutes, P=0.018). The incidence of perioperative complications (cardiac tamponade, etc.) was similar between the two groups. After an average of 2 years of clinical follow-up, only 3 patients in the conventional group had a small residual leak of ≤3mm (P=0.232). Survival analysis of AF recurrence Kaplan-Meier estimates showed the incidence of atrial arrhythmias between the two groups (minimalist group 13/65 (20.0%) vs traditional group 18/63 (28.6%), P=0.964), the incidence of stroke (0/65 vs 3/63 (4.8%), P= 0.232), and the incidence of major adverse cardiovascular events (7/65 (10.8%) in the minimalist group %) vs the traditional group 11/63 (17.5%) P= 0.502) was not significantly different. Preliminary research results show that compared with the traditional one-stop operation, the minimalist one-stop operation can shorten the operation time, and also has considerable perioperative safety and long-term prognosis.
