The patient and his family did not give up and were transferred to the Second Hospital of Zhejiang University.
The examination found that the serum creatinine exploded to 1718 (normal reference value <132). After understanding the condition, the initial diagnosis was acute renal failure.
Ma Kunling, director of the Department of Nephrology of the Second Hospital of Zhejiang University, carefully reviewed the medical records of other hospitals. The results showed that the renal tubules showed moderate-severe pathological changes. The tubular epithelial cells had vacuolar degeneration, shedding, necrosis, renal interstitial edema and inflammatory cell infiltration.

It is obvious that this is acute renal failure caused by acute tubular necrosis, and it is most likely related to drugs. Repeatedly asking the patient about the medical history, we made an unexpected discovery – 3 months ago, the uncle took the antipyretic acetaminophen because of a “cold”. After taking it for 1 week, he developed anorexia and abdominal distension, but he didn’t care at that time. After the symptoms gradually increased, and chest tightness, shortness of breath, oliguria, to the local hospital for treatment.
The pathological damage to the kidneys was clarified. Since there was no major problem with the glomerulus, Dr. Ma Kunling judged that the kidneys might be saved. On the basis of maintaining hemodialysis, the nephrology team of Zhejiang University Second Hospital gave glucocorticoid therapy. After a period of hospitalization, the patient’s urine output finally increased, and soon a polyuria phase occurred, and he was removed from hemodialysis treatment. Two months later, when I came to my clinic for a re-examination, my serum creatinine had dropped to about 100, and other indicators had basically returned to normal.
