Xiao Zhao is a native of Henan and works in Han. He was admitted to the general surgery department from the emergency department due to “prolapse of anal mass in the bowel for 2 hours”. The doctor took a closer look and found that the prolapse was rectal mucosa and frenulum, about 12cm x 15cm x 15cm in size. The surface mucosa was red and oozing blood was visible. Xiao Zhao was diagnosed with rectal prolapse. Anorectal surgery, gastrointestinal surgery and general surgery worked together, and Yao Lei, director of general surgery, formulated a treatment plan. On the morning of July 15, Yao Lei led the surgical team to perform transanorectectomy + ileostomy + laparoscopic intestinal adhesion release for Xiao Zhao. The operation was successful and the patient was transferred to the ICU after the operation.

The biggest cause of rectal prolapse is congenital hypofunction of the anal sphincter, which leads to weak sphincter strength. It may also be caused by the disorder of anal contractile function. Sometimes, when the patient has symptoms such as constipation, diarrhea or chronic cough for a long time, It can also cause the rectum to prolapse.

If the prolapsed part is only the rectal mucosa, it is called partial prolapse. If the bowel wall is full thickness, it is called a complete prolapse.

Rectal prolapse is more common in children and middle-aged and older women. Rectal prolapse in children is mostly mucosal prolapse, which usually heals before the age of 5. Can rectal prolapse in adults heal on its own?

Early rectal prolapse can be retracted into the anus by itself. As the disease progresses, the prolapsed mass must be recovered by hand. In long-term or advanced cases, both bowel and bladder can be incontinent.

Children’s rectal prolapse is mostly self-healing, so non-surgical treatment is the main treatment. That is, with the growth and development of children, the formation of sacral curvature, rectal prolapse will gradually disappear.

The treatment of adult complete rectal prolapse is mainly surgery, because unlike young children, the adult’s physical growth has stopped, and the possibility of rectal prolapse self-healing is very small.

What are the main clinical symptoms of rectal prolapse?

❶Early stage: the mucous membrane protrudes from the anus after defecation, which can be retracted on its own; later, it cannot recover on its own, and it needs to be reset by hand. Afterwards, there is a feeling of falling and incomplete defecation, and the frequency of defecation increases.

❷ Late stage: Prolapse of the rectum can be prolapsed after coughing, sneezing, walking, standing for a long time or with a little force. After prolapse, local swelling and pain can also be felt in the lumbosacral region, and the prolapsed mucosa has mucus Secretion, the mucous membrane is often stimulated to cause congestion, edema, erosion and ulcers, secretion can be mixed with bloody mucus, stimulate the perianal skin, and cause itching.

❸Incarceration: Due to the relaxation of the anal sphincter, incarceration rarely occurs. Once incarceration occurs, the patient will feel severe local pain, and the tumor cannot be reset with a hand support, and swelling will appear soon after prolapse of the anal canal. Hyperemia, mucosal folds disappeared. If left untreated, strangulation and necrosis can occur.

How can rectal prolapse be prevented?

1. Pay attention to the law of life and reasonable diet.

2. Develop the habit of regular bowel movements to prevent dry stools and avoid prolonged squatting when going to the toilet.

3. It is recommended to take a hot sitz bath after defecation and before going to bed to stimulate the contraction of the anal sphincter and have a positive effect on preventing rectal prolapse.

4. For patients with habitual constipation or difficulty in defecation, in addition to eating more fiber-containing foods, they should actively take measures to improve constipation, and do not use excessive force during defecation.

5. Diarrhea or enteritis should be treated in time to avoid delaying the disease.

6. Women should pay attention to adequate rest after natural childbirth to protect the normal function of the anal sphincter. Uterine prolapse and visceral prolapse should be treated in time.
