What are the five key tasks?

Carry out early screening, prevention and intervention for key diseases of the elderly, and strengthen early screening, intervention and classification guidance for hypertension and diabetes in the elderly. Continue to promote the health promotion of the elderly and popularize healthy lifestyles. Encourage each county (city, district) to set up a psychological assistance hotline for public welfare services for the elderly and guardians.
The second is to facilitate the elderly to see a doctor, increase the supply of geriatric medical services, guide a group of second-level and below hospitals in areas with rich medical resources to transform into geriatric hospitals for the elderly, and promote the construction of geriatrics clinical specialty. Explore the development of home medical services for the elderly, and innovatively carry out “one-stop” online services for home follow-up consultations for some common diseases and chronic diseases of the elderly.
The third is to vigorously develop rehabilitation nursing services for the elderly, strengthen the construction of rehabilitation medicine departments in general hospitals above the second level, promote the application of appropriate Chinese medicine rehabilitation techniques, and improve rehabilitation capabilities. Promote innovative and diversified elderly medical care service models, and support qualified medical institutions to carry out “Internet + nursing services” and extended services.
The fourth is to accelerate the construction of a demonstration province integrating medical and elderly care services, rationally distribute the medical and elderly care service network, and support the construction of a number of medical and elderly care service centers. Launch the “Health Knocking Action” for disabled seniors, and provide “Three Ones” free door-to-door health services for disabled seniors who apply. Improve the operation mechanism of medical care service payment guarantee and talent incentive.
Fifth is to improve the palliative care service capacity, promote the construction of palliative care centers in medical institutions, encourage qualified medical institutions to set up palliative care wards or beds, and through a multidisciplinary collaboration model, provide palliative care for the end-stage disease. Elderly patients shall provide pain and other symptom control, palliative care, psychological support and other services, and promote the establishment of a price system, drug allocation, capital investment and other institutional guarantees.
