For patients with underlying diseases, lying flat is a disaster, greatly shortening their survival time

I chatted with an out-of-town classmate yesterday and learned that he had been rescuing a patient the night before. Fortunately, the final outcome was not bad.

The patient he rescued was a 60-year-old man with a history of nasopharyngeal carcinoma 12 years ago, who was cured by radiotherapy and chemotherapy. The history of colon cancer surgery 2 years ago, and the regular review after the completion of postoperative chemotherapy were all normal. He has a 30-year history of smoking, a history of carotid artery occlusion stent implantation, and a history of hypertension. Although there are many underlying diseases, this patient usually has a relatively high quality of life. He occasionally plays cards and fishes in the sky.

However, 2 days ago, the patient slipped and fell into the water while fishing. Although he did not choke on the water, he was soaked all over. He started to have fever, cough, and asthma the next day. In the hospital, CT showed that both lungs were infected, and received oxygen inhalation, anti-inflammatory and anti-asthmatic treatment in the respiratory department. Unexpectedly, his condition gradually worsened at night, and developed into orthopnea and respiratory failure. After some rescue, he was put on a non-invasive ventilator, and his condition gradually stabilized the next day.

This can’t help but remind me of the recent Shanghai epidemic. There are always two different voices on the Internet, one is “lay down” and the other is “cleared”. The people lying flat have counted the data in Shanghai, and believe that although the number of infected people is huge, the severe rate is very low, and the large-scale closure of the city is not worth the loss, and the economic impact is too great. In fact, there are not a few people who hold this idea, including some well-known scholars and professors.

You must know that there are only 10 severe cases of 300,000 confirmed cases in Shanghai. This situation is the result of active anti-epidemic efforts. With the increase in the number of infected people, there will be more severe cases in the future. At present, an unvaccinated man in his 30s has developed severe disease. If it is released, a large number of patients with underlying diseases will be infected and even take their lives.

Imagine that there are 1.4 billion people in the country, 90 million COPD patients, and more than 10 million cancer patients (including new and past). If these patients are infected with the new coronavirus, If the already poor lung function and immunity are further reduced, the severe rate and mortality rate will inevitably increase. The above patient is only suffering from lung infection due to cold, leading to respiratory failure. If he is infected with the new crown, the chance of being rescued will be very slim. And how many elderly people with low immunity, many basic diseases, or over 80 years old are there in the whole country. And how many unvaccinated children and elderly people are in the country. They are in the whirlpool of the epidemic, and they are afraid that they may be infected at any time. Therefore, “lying flat” is a fallacy. Shouldn’t every life be cared for and respected? In China, which has a huge population base and a serious aging population, only “clearing” is a wise move.

A medical run after “lay flat” would be a disaster. As everyone knows, every year during the flu season, not only specialized hospitals such as thoracic, pulmonary, and children’s hospitals, but almost all hospitals’ emergency departments and respiratory departments are overcrowded. , that kind of scene is unimaginable. All hospitals will become hotbeds of Omicron. Patients, family members, and medical staff have become mobile sources of infection. The condition of the original inpatients has become worse, and medical resources have been exhausted. I don’t think there is any one The hospital was able to cope with this mess.

The central government has always adhered to the policy of “dynamic clearing”, which must be strictly considered. The economy is important, but the temporary stagnation is for once and for all stability. To borrow a famous line from the movie “South and North War”: “Today’s big step back is for tomorrow’s big step forward”!
