TCM syndrome differentiation and treatment of children’s enuresis

Enuresis refers to urination during sleep and wake up later, so it is also called bedwetting, and it is a common disease in children. Enuresis can also occur in frail adults.

Many diseases such as congenital bladder sphincter hypoplasia, recessive spina bifida, urogenital malformations, urethritis and other local irritation of the urethra, parasitic Insect disease, spinal or craniocerebral injury, brain hypoplasia, malnutrition, physical weakness after illness, etc., can all cause enuresis.

Urination is controlled by the voiding center of the brain. When the urine reaches a certain volume in the bladder, the pressure in the bladder rises sharply, and a urge to urinate occurs in the brain. This strong sensory impulse is sent to the spinal cord, and the reflex center sends out motor impulses, causing the detrusor muscle to contract and the external sphincter muscle to contract. And the perineal muscles relax, urine is discharged from the bladder. Enuresis can be caused by congenital brain hypoplasia or brain dysfunction caused by various acquired factors, or due to the disappearance of the reflex arc of the spinal cord, or the stimulation of chronic lesions in the urinary system and its surrounding tissues.

Because the infant’s central nervous system has not yet fully developed, it cannot control the bladder sphincter, so infant enuresis is a normal physiological phenomenon. Some children, due to excessive play during the day, are tired, and sleep soundly at night, and occasionally enuresis. Only after the age of three or four, it is morbid, called enuresis, and needs to be treated.

Traditional Chinese medicine believes that the occurrence of enuresis is mostly caused by insufficient kidney qi in children, deficiency and cold in the lower yuan, or weakness after illness, and deficiency of spleen and lung qi. photo, or caused by bad habits.

Acupuncture and moxibustion have good curative effect on enuresis.

Commonly used points for acupuncture: Guanyuan, Sanyinjiao. Alternate acupoints: Baihui, Qihai, Zhongji, Yinlingquan, Shenshu, Pishu, Zusanli, Lieque. Manipulation: With moderate or strong stimulation, when acupuncture at the abdominal acupoints, the acupuncture sensation (soreness and numbness) reaches the genitals. 10 to 15 days is a course of treatment.

Commonly used points for moxibustion: Baihui, Mingmen, Guanyuan, Zhongliao. Take 2~3 points every day and smoke with moxa sticks for moxibustion.

Auricular acupuncture therapy: commonly used points: sympathetic, brain, kidney, bladder, pillow, urethra to find sensitive tender points. Once a day or every other day, keep the needle for half an hour. After taking effect, acupuncture every five days to consolidate the curative effect.

Traditional Chinese Medicine for Enuresis:

①Bed-wetting belongs to “insufficiency of kidney qi”, which can be seen as pale complexion, mental retardation, weak waist and legs, clear and long urination, and even cold limbs, aversion to cold, and a slow and weak pulse.

Fang 1: 2 grams of deer antler powder (to be blistered), 3 grams of ginseng (to be chewed), 20 grams of mulberry and octopus, 10 grams of calamus , Polygala 10 grams, Angelica 12 grams, keel 30 grams, 30 grams of oysters. Decoct in water, take one dose every two days (this is the dose for five-year-old children).

Fang Er 2: 30 grams of Astragalus, 12 grams of Shayuanzi, 10 grams of Goatwood meat, 10 grams of Dodder, 10 grams of Schisandra chinensis, Bugu 10 grams of fat, 10 grams of chive seeds. Decoct in water, take one dose within two days (this is the dose for a five-year-old child).

②Enuresis during sleep, which belongs to “deficiency of spleen and lung qi”, accompanied by fatigue, weakness of limbs, loss of appetite, loose stools, pale tongue , slow pulse.

Prescription: Yam 30g, Taiwu 10g, Yizhiren 10g, Codonopsis 30g, Atractylodes 12g, Poria 10g, Schisandra 15 grams, simmer in water, take one dose every two days.

③Scientific research has shown that in addition to strengthening the body and strengthening the body, the treatment of enuresis with the method of promoting blood circulation and refreshing the brain can often be effective. Satisfactory curative effect was received.

Fang Yi (Huoxue Xing Nao Method, Huangma Jiuxiang Tablets): 100 grams of astragalus, 75 grams of horse money (first fried with hemp until browned), 125 grams of nine incense insects, 45 grams of Chuanxiong, and 35 grams of calamus. Grind them into fine powder, mix well, and make into tablets, each tablet weighs 0.5 grams. Take two tablets every night before going to bed for one month.

Fang Er (Fuzheng Gumu Method, Three Pieces of Astragalus), Astragalus 100g, Gorgon 125g, Golden Cherry Seed 60g, 50 grams of raspberry, 50 grams of dodder, 60 grams of yam, and 25 grams of licorice. Grind them into fine powder, mix well, and make into tablets, each tablet weighs 0.5 grams, 6 tablets each time, three times a day for one month, and “Huangma Jiuxiang Tablets” every night.

In these two prescriptions, in addition to the commonly used drugs for treating nocturia, invigorating the spleen and nourishing the kidney, invigorating the spleen and nourishing the qi, and solidifying the urine , also added nine incense insects, Shichangpu, horse money, Chuanxiong. Nine incense worms have strong effects of invigorating the spleen and kidney, and strengthening Yuanyang: Shichangpu has the functions of clearing the nine orifices, stopping urination and slightly stimulating the spirit; Strychna has the effect of treating wind paralysis, bladder paralysis, and enuresis; Chuanxiong is the qi medicine in the blood, which has the function of removing blood stasis.

One-Shot Prescription :

(1) Knock a small hole in the raw egg, add seven white peppers, and plug the hole in the eggshell , steamed, eat 2~3 eggs every night before going to bed, do not drink water.

(2) Psoralea 60 grams, Remove impurities, fry in a pot for 10~20 minutes, make a popping sound, take out and let cool, grind into fine powder. 120 grams of yam, dry, grind into powder, mix with psoralen powder, swallow with a small amount of boiling water, each time Take 10 grams, once in the morning and once in the evening. Both psoralen and yam are nourishing and strong products, and the cost is low, so they can be taken for a long time.

( 3) One piece of chicken intestines, one piece of Ziheche, put it on a tile and dry it with slow fire, grind it to a fine powder, take 6 grams each time, swallow it with boiled water, and take it once in the morning and once in the evening.

(4) 12 grams of Yizhiren, 6 grams of ephedra, 15 grams of mulberry octopus, 10 grams of calamus. Decoction in water.

(5) 15 grams of mulberry octopus, 15 grams of astragalus, 15 grams of keel, 3 grams of cinnamon. Decoct in water, 1 dose per day (this is for children aged 7 to 12 years old) dose).

(6) Psoralea 5g, Cooked Nutmeg 5g, Evodia 5g, Schisandra 5g, Yizhiren 5 grams, 1 pig bladder (washed). Add the above traditional Chinese medicines into the pig bladder, tie its mouth well, use a thick needle to prick the pig bladder several holes, put it in a basin, add 1.5 kg of water, and boil for about an hour , remove the medicine residue, drink soup, and slice the pig bladder. Adults eat it at one time, and children eat it in 2~3 times.

( 7) 3 sticks (about 2 inches long) of scallion whites and 30 grams of sulfur, mashed together like mud, apply the medicine on the navel when the child sleeps, bandage it with gauze externally, and remove 8 to 10 hours later.
