risk! The 1.5-meter-long, 1cm-thick steel bar penetrated the man’s body from the armpit, only 2-3cm away from the large blood vessels

At 15:20 on July 26, an accident occurred at a construction site. Mr. Wu, 29, was accidentally thrust into his body with a crowbar about 1.5 meters long and about 1 cm in diameter. First line! At 15:35, 120 emergency personnel and the fire rescue team arrived at the scene. After preliminary treatment, they jointly sent the patient to the hospital for treatment, and notified the emergency department of Changsha First Hospital in advance through the pre-hospital emergency system to prepare for rescue.

At 15:51, the patient arrived at the hospital, and the trauma team immediately opened the green channel and quickly began rescue, giving ECG monitoring, oxygen inhalation, establishment of bilateral venous access, rapid fluid replacement, analgesia, and maintenance of the patient’s vital signs Stabilize, start the emergency blood transfusion process, prepare for emergency surgery, and everything is proceeding in an orderly manner.

At this moment, a difficult problem is in front of the patient, whether there is damage to the large blood vessels and important organs, and it is necessary to improve imaging and other related examinations immediately. The patient’s external steel bars are long, so he can barely sit, and cannot complete CT examination and surgical operations. If the diagnosis and treatment time is delayed, traumatic shock or respiratory arrest may occur at any time.

Led by the medical department, the decision was made after joint consultation by a multidisciplinary team of trauma experts including emergency department, vascular surgery, thoracic surgery, anesthesiology, imaging medicine, blood transfusion, spine surgery, and critical care medicine. In the case of adequate pain relief, blood preparation, and preparations for emergency tracheal intubation, emergency surgery, etc., fire rescue personnel are asked to shorten the external crowbar, and then immediately complete imaging examinations such as chest and abdomen enhanced CT.

At 16:10, fire rescue personnel in Hunan Province used hydraulic pliers to cut off the crowbar while fully protecting the patient, which did not cause secondary injury to the patient.

At 16:15, the patient was accompanied by medical staff to complete the CT examination. The examination results showed that the left axillary foreign body penetrated through the thoracic cavity, behind the cervicothoracic cone and into the subcutaneous soft tissue of the right neck and back. Fortunately, the patient’s aorta and heart were not injured, and the steel bar was only 2-3cm away from the aorta.

At 16:37, the patient was rushed into the operating room. On the operating table, the thoracic surgeon carefully separated the lung tissue and subcutaneous tissue adhered to the crowbar, repaired the tissue damage caused by the crowbar, and carefully sutured the bleeding. After more than two hours of emergency surgery, the patient was finally successfully pulled from the brink of death. He was sent to the intensive care unit for treatment after surgery.

Zuo Qi, deputy chief physician of the trauma center, said: The key to successfully rescuing this patient with penetrating steel bar is the seamless connection of 120 emergency services in the front and the hospital. The multi-disciplinary team of the center has complementary technologies, the smooth and efficient emergency green channel, and the strong support of the fire department. Next, the hospital experts will continue to give full attention to the patients’ postoperative conditions, promote recovery, and strive to discharge the patients as soon as possible.
